To get a head start you can complete these forms and upload them to your portal, once created.

- Please send a copy of the front and back of your health insurance card.
- A copy of photo ID (driver’s license works)
- You may consider uploading the following which would help in the treatment.
- A list of your current medications with the dosages.
- A list of past psychiatric medications or any treatment record.
- A list of medical problems you have and treatments you have received.
- The first appointment is about 45 mins. You have the option of having your appointment via video or in office at either or Sacramento office.
